TECHNICAL GUIDE FORM FILLING PERIOD VAT SPT 1111

1 . This format has been in accordance with the Regulation of the Director General of Taxation Number Per-44/PJ/2010 On Form , Fill and Filling Procedures as well as submission of the VAT return period Hints Along with filling and can only be used for charging VAT return period ranging tax period January 2011 onwards .

2 . Form 1111 VAT return period consists of 1 Master Pages and Page 6 Appendix with the following information :
a. Parent Form VAT return period in 1111 ( the Notice Period Value Added Tax ) with code F.1.2.32.04
b . Form 1111 AB ( recapitulation Delivery and Acquisition AB or Form 1111 ) with code D.1.2.32.07
c . Form 1111 A1 ( List Export BKP Tangible , Intangible BKP , and / or JKP ) with Code D.1.2.32.08
d . 1111 Form A2 ( Output Tax Register Handing Over of Domestic Tax Fatur ) with Code D.1.2.32.09
e . Form 1111 B1 ​​( Can Creditable Input Tax upper Import BKP BKP and Utilization of Intangible / JKP From Outer Regional Customs ) with Code D.1.2.32.10
f . Form 1111 B2 ( Can Creditable Input Tax upper Acquisition BKP / JKP Home Affairs ) with Code D.1.2.32.11
g . Form 1111 B3 ( Input Tax Register That Can not Be Credited or That Got Facility ) with Code D.1.2.32.12

3 . SPT result of this printing must be signed , stamped with the company and should not be folded or crumpled . To print using only the HVS paper size :
a. Folio / Government Legal ( 8.5 x 13 inch ) ;
b . Minimum weight 70gr ;

Just A Dream

There I was again tonight. I invited to join a party with all my beloved friends. I was happy since I can meet my old friends there. On my way, it has gone happily but, then, when I was there, everything wasn’t as well as what I have imagined. I was forcing myself to laugh with faking smiles. There was full with walls of insincerity. Then when all have been same old tired, I was shifting my eyes and I found a vacancy of love. I couldn’t enjoy everything but him. What I have felt before, it vanished when I saw his face. All I can see is that it was enchanting to meet him. It just like his eyes whispered “have we meet?”. Ohh God! his sillhouette crossed the room, and starts to make its way to me, then I founded in a lonely place.

“Hi, I’m Jhonatan, what’s your name?”
“Oh, my name is Taylor. Nice to meet you”
“What’s a great name. Nice to meet you too, taylor”

There was only both of us, then the playful conversation starts. I was countering all his quick remarks like passing notes in secrecy. Haha, what a great conversation. All I can say is I was enchanted to meet him. This night is sparkling and I don’t let it go. At 2 AM one game question kept me up, “who do you love?” I wonder till I’m wide awake.

I was wonderstruck with blushing all the way home. He made me crazy of love. His face is running up on my mind and I couldn’t throw it out. It is what we called as love, isn’t it? I know I’m pacing back and forth. In my life, he is the best boy that I ever met. My thoughts will echo his name until I see him again.

3 AM, I was in my own house. I was so sleepy then I went to my bed to have a sleep. Before I closed my eyes, I wish to meet him again, someday. “Tokk.. tok… tok…” I heard someone knock my door, that wake me up. I went to my door to open it. And you know, I don’t believe it, He was at my door. I would open up.

“Hey” he said.
I was getting stuck and I couldn’t speak.
“May I enter?” asked him.

We were sitting on ottoman.

For my EPD 397 project, please grant me permission to study the way in which credit has been awarded for the discovery of nuclear fission. Although Otto Hahn received the 1946 Nobel Prize in Chemistry for the discovery, several people assert that Lise Meitner and Fritz Strassman should have also received credit. In my research, I will attempt to discern how credit should have been bestowed.
This topic meets the criteria for a successful topic in this course. First, I am interested in the topic. As a nuclear engineering student, I realize that the discovery of nuclear fission was perhaps the single most important discovery this century in my field. As a woman scientist, I am also deeply interested in the successes and challenges faced by other women scientists. A second way in which this topic meets the criteria is that it can be quickly researched. A computer search in the library has revealed many sources available on this topic. Attached to this memo is a summary of one such source, Lise Meitner: A Life in Physics by Ruth Sime.
This topic also meets the third criterion for a successful topic in this course, namely, that it be technical. The fission of a uranium nuclear involves an understanding of both chemistry and physics principles. By focussing on this single discovery, I believe that I can achieve the fourth crierion for a successful topic: the achievement of depth. Finally, because the library system at the University of Wisconsin offers such a wide array of possible sources, including papers in German, and because many of these sources have been written for audiences more technical than my intended audience, I believe that I can create a project that is unique.

Parts of a letter
The following are the parts of a business letter listed in the proper sequential order.

1. Letterhead - Stationary printed at the top of the page including the company name, logo, full address, and other elements such as trademark symbols, phone & fax numbers, and an e-mail.
2. Dateline - The date is the month (spelled out), day, and year. If you are using Microsoft Word, click - Insert, then Time and Date. Press Enterfour times after the date.
3. Letter Address - The complete address of the recipient of the letter. The letter address usually includes the personal title (Mr., Mrs. etc.), first and last name followed by the company name, street address, city, province, and postal code. Press Enter twice after letter address.
4. Salutation - The word Dear followed by the personal title and last name of the recipient (Dear Mr. Smith). Press Enter twice after the salutation.
5. Body - The text that makes up the message of the letter. Single-space the paragraphs and double-space between the paragraphs. PressEnter twice after the last paragraph.
6. Complimentary closing - A phrase used to end a letter. Capitalize only the first letter. If there is a colon after the salutation, there must be a comma after the complimentary close. Press Enter four times (or more) after the complimentary close to allow for a written signature.
7. Name and title of writer - Type the first and last name of the sender. The sender's personal title (Mr., Ms., Dr., etc) should be included. Use a comma to separate the job title if it's on the same line as the name. Do not use a comma if the job title is on a separate line. Press Enter twice after the name or title.

MY LIFE START HERE

Nama saya M.Ressofi Kuswan, aku lahir di Bekasi tepatnya pada tanggal 19 November 1990. Aku anak bungsu dari 4 bersaudara, 2 kakak laki-laki dan 1 kakak perempuan. Ayahku bernama Sofwan Abdul Ghani, berasal dari kota Brebes. Dan ibuku bernama Kuswiyah, berasal dari kota Purwokerto. Kami tinggal sekarang di kota Bekasi, setelah sering pindah-pindah rumah sebelumnya di Jakarta.
Saat masih kecil aku sering diasuh oleh saudara ibu aku, karena ibu dan ayah saat itu sangatlah sibuk dengan pekerjaannya masing-masing. Mungkin karna aku jarang ketemu ibu saat aku masih balita, aku jarang mendapatkan asupan Asi dari ibu. Dan mulailah kebiasaan yang agak terbilang aneh, aku suka mengempeng jari jempol kananku. Tapi itu tidak berlangsung lama, aku tinggalkan kebiasaan itu saat duduk dibangku Sekolah Dasar.
Kata ibuku, aku terbilang anak yang paling lama didalam kandungan. Sebab kandungan yang biasanya itu tidaklah lebih dari 9 bulan. Tetapi aku memang terbilang cukup lama 13 bulan didalam kandungan seorang ibu. Ibuku sangatlah hebat, ia bisa menahan beratnya sebuah kandungan begitu lamanya. Jika dibawa kerumah sakit, mungkin ini akan dilakukan sikap siaga seperti disesar. Tetapi ibuku tidak ingin melakukan sesar, ia tetap berserah diri kepada Sang Ilahi Rabbi agar kandungan ini tetap normal walau lama sekali didalam perut seorang ibu. Dan alhamdulillah dengan karunia Allah, aku terlahir normal dengan wajah nan rupawan.
Saat berusia genap 6 tahun, aku memulai pendidikanku di SDN Pejuang I. Aku tidak memulainya dari Taman Kanak-Kanak. Karna mungkin saat itu dipikiran ibu aku, sama saja sekolah langsung SD atau tidak. Jadi ibu aku langsung memasukkan aku ke Sekolah Dasar. Setelah lulus dari Sekolah Dasar, aku melanjutkan pendidikanku di pondok pesantren. Tepatnya di Pondok Modern Darussalam Gontor yang bertempat di Kota Ponorogo.
Aku mengemban pendidikanku di pondok pesantren selama 6 tahun lamanya, dan ditambah dengan pengabdianku selama 1 tahun. Dalam genap 6 tahun aku belajar dipesantren Gontor 1, aku sempat dipindahkan menjadi kader ke pondok cabang yaitu Gontor 5 Darul Muttaqien yang bertepat di Kota Banyuwangi Jawa Timur. Dan pengabdianku ditempatkan pula di Gontor 5 Darul Muttaqien.
Keinginanku saat masih duduk dibangku SD adalah memang ingin meneruskan dipondok, karena sudah teriming-imingi oleh ayahku yang menjabarkan bagaimana kehidupan di pondok pesantren. Setelah lulus SD aku mendaftarkan diri dipondok Gontor 2 menjadi calon pelajar. Disana aku dididik dan ribuan calon pelajar lalu akan diseleksi siapa sajakah yang pantas meneruskan menjadi kelas 1 dipondok Gontor 1 atau cabangnya.
Akhirnya setelah lulus ujian seleksi, aku ditempatkan dipondok Gontor 1 yang tidak jauh dari pondok Gontor 2. Aku duduk dikelas 1D dan tinggal diasrama Gedung Baru Shigor yang mana 1 kamar ditempati kira-kira 40 santri. Disana aku dididik tentang kedisiplinan oleh para pengurus yang mana masih duduk dikelas 5 KMI(Kulliyyatul Muallimiinal Islamiyah).
Aku mendapatkan banyak teman dari berbagai macam kota hingga luar negeri. Dan seterusnya aku lulus ke kelas 2 dan sudah menjadi anak lama dipondok Gontor tersebut. Semakin lama tinggal dipondok aku semakin terbiasa dengan lingkungannya nan damai. Walau banyak peraturan tapi asal kita jalani saja, semua terasa seperti biasa saja tanpa ada beban didalam hati.
Aku dulu sempat merasakan hukuman yaitu dibotak kepalanya karena telah memalsukan tanda tangan saat ingin piket kelas. Berawal dari kemalasan aku yang memang tidak ingin membersihkan kelas dipagi hari. Peraturannya adalah membersihkan kelas dengan menyapu dan lain-lain lalu setelahnya harus absensi karna akan ketahuan kelas manakah yang belum disapu. Tetapi aku tidak menyapu malah hanya absen saja, dan ternyata bagian pengajaran yang dikelola oleh para ustadz mengecek kelas disemua gedung dan mendapatkan kelas aku yang kotor tetapi sudah tertanda tangani. Akhirnya saat kelas dimulai, Ustadz KMI sudah menunggu kedatangan aku dikelas dengan membawa gunting. Dan aku pun datang ke kelas, tanpa basa-basi rambut aku dipotong acak-acakan ditempat dan disuruh untuk membotakkannya ditukang cukur. Dan rambutku sudah botak tanpa sehelai sedikit pun lalu menutup dengan sebuah peci/kopyah.
Saat duduk dikelas 4 aku pernah kabur dari pondok lantaran bujukan seorang teman yang berpura-pura mengalami sakit dalam dan ingin berobat dirumah agar dapat diurusi oleh keluarga. Dimulai pada hari jumat pagi, saya, Muhtarom dan Dira teman dekat yang tinggal didaerah bekasi juga ingin keluar pondok dihari libur ini. Setelah mencoba ijin keluar pondok kami tidak diijini lantaran asalan yang tak jelas mungkin. Tetapi akhirnya kita bertiga tetap berangkat tanpa ijin oleh Pengasuhan Santri. Kami bertiga jalan-jalan ke telaga ngebel yang masih bertempat didaerah Ponorogo. Sesampainya disana kami bersenang-senang dengan berenang di tempat jalannya air ke telaga. Udara di telaga memanglah begitu sejuk dan dingin, setelahnya berenang kami pun makan siang.
Dan hari semakin sore, sudah saatnya kita harus pulang ke pondok. Tetapi kita kehabisan uang saku dimana hanya tertinggal 21ribu rupiah dan tidak cukup untuk tiga orang pulang kembali lagi kepondok. Akhirnya kami memutuskan untuk si Dira saja yang pulang dan esoknya harus kembali dengan meminjam uang yang banyak agar kami berdua yang masih tinggal ditelaga bisa ikut pulang juga ke pondok lagi. Kami berdua memegang uang 10ribu dan 11ribunya dibawa Dira pulang ke pondok. Hari semakin malam dan udara begitu mencengkrang hingga masuk ke tulung rusuk yang paling dalam. Setelah menunaikan salat isya berjamaah disalah satu masjid dekat telaga, saya hendak mencari makanan untuk makan malam kita berdua. Dan turunlah hujan rintik-rintik sehingga membasahi tubuhku yang sudah sangat kedinginan ini. Aku menemukan mie ayam yang dekat saat kami makan tadi siang, dengan menggenggam uang 10ribu aku pun bertanya kepada bapak-bapak yang berjualan. Kataku : “ Pak, mie ayam berapa harganya”, lalu bapaknya menjawab : “ 2 ribu rupiah le”.
Aku pun terkegut begitu murahnya harga mie ayam disini, sangatlah beda dengan harga mie ayam didaerah Jakarta dan sekitarnya. Memang harga dijawa begitu murah ketimbang harga-harga di Ibu kota yang begitu meninggi. Dengan muka yang begitu senang karna harga mie ayamnya murah aku pun langsung mengasih uangku, lalu tiba-tiba bapak penjualnya menggenggam bajuku yang basah dan berkata : “ Kamu tinggal dimana le?”, aku pun menjawab : “ Tinggal dimasjid seberang pak”. Dan kemudian bapak tersebut memperbolehkan aku tinggal diwarung rumahnya saja dan menyuruh aku memanggil teman aku si Muhtarom untuk datang ke tempat mie ayam tersebut.
Dengan rasa senang aku pun berjalan cepat menuju masjid dan memberitahukan Muhtarom untuk bermalam di warung bapak tersebut. Saat ingin beristirahat, kami dibekali 2 selimut setiap orangnya. Dan saat kita pakai 2 selimut tersebut, tetap saja dinginnya malam yang menyelimuti telaga begitu terasa walaupun kami berdua sudah memakai 2 selimut. Dan keesokan harinya kami dibangunkan untuk salat subuh, kami bangun jam 6 pagi tapi hari tetap saja masih gelap dan dingin saat kami ambil air wudlu. Aku tidak sanggup dengan dinginnya air disini tetapi mengapa warga disini biasa-biasa saja dengan kedinginan ini, mungkin karna mereka sudah terbiasa oleh lingkungan sekitar sini.
Dan akhirnya Dira pun datang kurang lebih jam 9 pagi, tetapi anehnya dia tidak membawa uang banyak lantaran belum dapat pinjaman banyak dari teman. Dengan terpaksanya kami pun nekat pulang dengan uang seadanya. Kami naik angkot dan tiba diterminal kami berjalan, kadang mengumpat saat kami melihat mobil pondok lewat. Dan kabar baiknya Muhtarom mempunyai satu kenalan di Ponorogo, akhirnya ia meminjam sepeda motor untuk memulangkan kita kembali ke pondok.
